Participant Assistance and Care (PAC)

Participant Assistance and Care (PAC) Services are provided to allow participants (consumers) with developmental disabilities to remain and live successfully in their own homes, function and participate in their communities, and avoid institutionalization. PAC services support and enable the participant in activities of daily living, self-care, and mobility with the hands-on assistance, prompting, reminders, support, and monitoring needed to ensure the health and safety of the participant.

A relative of the participant may be a provider of PAC. The decision that a relative is the best choice of persons to provide these services is a part of the person-centered planning process and is documented in the PCISP. When the provider is a relative, there is an annual review by the IST to determine whether the participant’s relative should continue to be the provider of PAC.

Required Contractor Information: Before engaging into a contractual agreement with the company, the contractor will submit the following documentation:

  • A current resume.
  • Original College Transcript.
  • Copies of diploma(s) showing qualification to provide behavioral services.
  • Copies of professional Licenses or certifications, if applicable.
  • Current CPR and First Aide Certification.
  • Copy of Driver’s License
  • TB test result (annual)
  • Three positive references.
  • Satisfactory local criminal history check in compliance with Indiana Administrative Code 460 IAC-6-10-5.
