Participant Assistance and Care (PAC)

Family Supports Waiver (FSW) only

Participant Assistance and Care (PAC) Services are provided to allow participants (consumers) with developmental disabilities to remain and live successfully in their own homes, function and participate in their communities, and avoid institutionalization. PAC services support and enable the participant in activities of daily living, self-care, and mobility with the hands-on assistance, prompting, reminders, support, and monitoring needed to ensure the health and safety of the participant.

A relative of the participant may be a provider of PAC. The decision that a relative is the best choice of persons to provide these services is a part of the person-centered planning process and is documented in the PCISP. When the provider is a relative, there is an annual review by the IST to determine whether the participant’s relative should continue to be the provider of PAC.

Get Connected To:

  • Inform teams (guardians, case managers, etc.) that you’re ready to begin services after hearing from Blueprint that all documentation is completed for onboarding.
  • Follow the Blueprint Policy and Procedures Manual instructions regarding the Oasis App.

What’s Next:

  • If needed, CPR (before beginning services)
  • Approximately 10 hours of Training Videos
  • Once completed, you will receive a payment of $200 on the following paycheck
  • Training in Documentation through state-mandated EVV app
